We believe hospitals and caregivers want to provide the best care and patient experience possible. No one purposefully intends for patients and their families to suffer the heart-wrenching loss of a loved one or injury from a medical mistake. The saying that “we’re only human, and humans make mistakes” is true. It's in our nature. Potential mistakes are there, waiting to happen for any number of reasons and circumstances, some minor, some not.

We also believe that patient harm is preventable. In fact, we know it. In our view, the worst thing about making a mistake is not learning from it and using it as an opportunity to do better in the future.

Our story is one of doing everything in our power to stop patient harm and the suffering that goes hand in hand with avoidable medical errors. That’s why our multi-disciplinary team is dedicated to using our collective expertise in safety, process improvement, and quality and risk management to help hospitals, large and small. Douglas B. Dotan, MA, CQIA
President | CEO

A visionary and driven advocate for patient safety, Douglas was an early pioneer in the field. As a former air force pilot and accident investigator, his vision was to apply his unique expertise of flight safety processes and the impact of human error to the world of healthcare. During his time in aviation, he designed and implemented cutting-edge, process-based quality management models based on data analysis, which reduced accident rates by 50 percent within a five-year period. In 2000, Douglas launched a private corporation to apply successful aviation safety methodologies and practices for healthcare safety reporting and the analysis of avoidable medical mistakes, and unsafe or hazardous conditions.

Douglas won the national AHRQ and the ONC (Office of the National Coordinator) Challenge 2.0 Award for Patient Safety software in 2012 with the creation of the Purple Button mobile app. He has continued to envision and create low cost, high value applications that enable healthcare facilities to reveal best practices for the creation and rapid sharing of institutional knowledge to improve quality and performance, prevent harm, reduce costs and elevate patient safety.

Dougles holds a Master's degree in Behavioral Science, Industrial and Organizational Pyschology from the University of Houston, Clear Lake and a Bachelorof Science, Geosciences from Bar-Ilan University, Israel.

Kerry Koski
Operations | Compliance

Kerry is a change leader with a significant record of accomplishment in helping companies achieve and sustain a competitive advantage. His interpersonal skills coupled with technical astuteness and a bias for action has consistently generated bottom-line results in environments ranging from small firms to Fortune 500 corporations. Kerry has coached senior leadership in industries which demand uncompromised commitment to safety/quality and a relentless pursuit of on-time delivery within budgeted cost.

His deep-rooted belief that ‘Companies Don’t Succeed, People Do’ has been a cornerstone in his career success. With a thorough understanding of the interrelationship between business management systems and human assets, Kerry is able to quickly assess strengths and weaknesses and cull the non-value added elements. He developed his deep operating expertise through years of hands-on training and implementation of continuous improvement methodology modeled after the Toyota Production System. He had the good fortune of being coached by members of the Shingijutsu organization (founding fathers of the Toyota Production System). His leadership strengths are exemplified in cultural transformation, behavior based change, and business process optimization. Kerry earned an MBA from the University of Texas and a Bachelor of Science from Purdue University.

AnnGail Smith
Risk Management | Product Specialist

With 20+ years of broad-based experience in risk management and hospital administration, AnnGail has an in-depth, insider’s understanding of healthcare operations, quality of care issues and all sides of the patient safety equation. She brings to bear significant knowledge and expertise from serving at the University of Texas Medical Branch, an esteemed 600+ bed tertiary care teaching hospital with on and off-site clinics. With responsibility for enterprise-wide risk policy and procedures, including education and training, implementation and monitoring, JCAHO compliance and quality assurance, AnnGail has first-hand knowledge of the daily challenges of healthcare staff face at all organizational levels. She also has extensive experience in incident management, reporting and investigations and system design for acute care, long-term care and clinic settings.

From working closely with C-Suite administration to consulting with patient safety and quality managers as well as clinicians of all disciplines, AnnGail is adept at effectively communicating with all stakeholders to reach common goals. She has also held a variety of positions in professional societies, taken leadership roles for Patient Rights and has lectured in both quality assurance and risk management at the local, state and national levels. She holds a Bachelors of Arts from the University of Illinois Champaign and a Master of Arts from the University of Illinois Congress Circle.

Karen Smith
Market Strategist | Communications

A results-driven strategist and communications consultant, Karen has consistently helped clients define their vision, position in the marketplace and create growth. Adept at project management and providing strategic guidance, Karen is comfortable working closely with all levels from senior executives to operations. She has a proven record with small business owners and Fortune 500 corporations, helping clients penetrate difficult market segments and securing more than $300M in contracts. Karen believes research and analysis are the keys to clearly seeing the horizon’s edge in order to develop effective strategies for market growth.

Karen has a background in international security and global risk management. As part of a two person start-up team, she helped establish, ASI Group, a subsidiary that was acquired three times and is now owned by United Healthcare Global. She provided intelligence briefings, overseas protection services and managed a network of global agents to ensure the safety of high-profile Fortune 500 executives, World Bank/IMF assets, diplomats, celebrities and corporate aviation crews. Karen developed the company’s strategic vision and participated in a wide range of functions from marketing to product creation. She created a successful line of risk management products that included World Watch®, Hot Spots® and Airline Insider®. Karen was published several times and attended annual OSAC briefings at the U.S. Department of State. She holds a Bachelor of Science, Geophysics from the University of Houston.

Grace Ku
Product Support Analyst

Grace earned her Health Information Technology certification from the University of Texas at Austin. Her specializations included health informatics; operational models of healthcare practice; workflow, process redesign and project management; electronic health records and health information exchange. She also completed hands on training with six EHRs. Grace became interested in healthcare after a successful career in restaurant management. She also holds a Bachelor of Science degree in Hotel and Restaurant Management from the University of Houston – Conrad N. Hilton College.
